Observation

During the course of this investigation it was determined that WAC 110-301-0345 was violated when YMCA staff failed to scan the enviornment on the day the children went missing before going back inside from outdoor play. Staff failed to listen for both verbal and nonverbal cues to anticipate problems and plan accordingly. Evidence gathered from the investigation also demonstrated that the staff were unaware of their count when going back inside. At the time the staff were looking for one missing child, the staff were unaware that a second child also had gone missing. The children were missing for about 45-50 minute. In addition, one of the children reported that this is the second time he is left outside without supervision. The first time he had to be let in the building by a janitor.
